In an ongoing effort to be fiscally responsible (and to make sure materials are only going out to those who desire them), the Mental Health Commission sends out an annual card asking recipients to check what materials they wish to continue to receive. You may also receive agendas and minutes by way of e-mail. Just indicate that desire below. Please write your e-mail address VERY CLEARLY, and remember, e-mail addresses are case sensitive so don't capitalize unless your e-mail address contains capital letters. Please place a checkmark in the box next to each Committee from which you wish to receive materials.
