Faculty

Residents work closely with faculty members in our residency program.

CCRMC residency program has over 50 faculty members that are directly involved with resident teaching. The faculty shown below are members of the Faculty Leadership Group, a diverse committee of teachers representing many disciplines throughout the residency program whose job is to assist the Residency Director in designing an ideal teaching curriculum.

Program faculty are full-time employees of Contra Costa County. Our "staff model" allows maximal teaching, avoids conflicts observed in programs where faculty salaries are largely derived from patient care revenues.

  • 60 faculty teach residents at least one half-day per week.
  • Each service has "registrars" - most are family physicians with additional training and experience such that they function as another specialist, whether OB, pediatrics, or surgery.
  • Registrar system allows for teaching always through the eyes of a family physician.
  • Simultaneously the resident gets "expert" knowledge through the eyes of the "specialist."
  • Faculty is culturally diverse.
  • Most specialties are represented at CCRMC.

Jeremy Fish, MD
Residency Program Director

It has been a deep honor for me to be the Residency Director at our vibrant and active Family Medicine Residency Program these past several years. I came to Contra Costa as a resident in 1991, did a one year OB fellowship at Contra Costa, and joined the Internal Medicine Registrar group in the summer of 1995. I recently shifted over to being a part-time surgery registrar and continue to actively teach in the inpatient setting. This year I am putting the bulk of my energy into the creation of our new Family Medicine Registrar positions—strong teaching roles that bridge the inpatient and outpatient settings. Beginning in July, 2009 we will have two new inpatient Family Medicine Services made up of medicine and surgery patients, which is a very exciting development further strengthening the role of family physicians in our already remarkable system.

I am deeply grateful for many things—most importantly my wife Susan and young son Benjamin, having a rich and rewarding career in a Family Medicine-strong environment with amazing teachers and more amazing residents, and running and reading which fill my energy tank every day.

Elise Lewis, MD

I graduated from Tulane University School of Medicne in 2001. I have been with Martinez since starting my residency here in 2001. After graduating from this program, I took a position in the Department of Family Medicine working in our small eastern-most clinic in the county in Brentwood, CA. I also work in the dermatology clinics as a dermatology registrar. I enjoy ambulatory medicine and intend to continue to focus on outpatient management of common illnesses and preventive medicine. I recently joined the Faculty Leadership Group and have a special interest in the ambulatory curriculum of the residency. Since January, 2007 I have also taken on the role of Associate Residency Director which has enabled me to further pursue my interest in resident education. When not at work, I enjoy spending time with my husband and sons.


Chris Farnitano, MD

I completed my Family Practice Residency training at Contra Costa in 1994. I fell in love with this program the day I interviewed here. I felt I had finally found a place where medicine was practiced the way it should be, and residents were taught to become "real doctors," with the skills and attitude to do the utmost possible for their patients. I was offered the opportunity to stay on as an active faculty member and have been here since. I have served as a member of the faculty leadership group and as assistant residency director. My current interests include HIV care, group medical visits, and emergency medicine. I teach residents in the Family Medicine Clinic, flex sig clinic, and ER. I have a beautiful wife Lisa and three boys, all of whom were born at Contra Costa Regional (two during my residency). My hobbies include hiking and world history. Stuart Forman, MD
Asst. Director - Critical Care Unit.

I have been on the faculty since my graduation from this program in 1993 and I stayed on because I loved working here and couldn't think of anywhere else I'd be happier. I am a registrar in surgery, gastroenterology, and critical care, and I am the assistant director of the ICU, but despite all this I still maintain an active outpatient family practice clinic. I am a strong advocate for residents learning procedures and I believe that family practitioners are limited only by their own expectations and imagination. I spend most of my time outside of work having fun with my daughter, but I also play banjo and guitar in a local bluegrass band, and I enjoy amateur astronomy, skiing, backpacking, and mountain biking when I can find the time. Ann Harvey, MD
Lead Preceptor

I came to Richmond Health Center in 1994 from the UCSF-SFGH Family Practice residency program. I love precepting the fantastic residents that this program attracts and serving our patients alongside my dedicated coworkers. I divide my time between Richmond Family Practice Clinic, HIV clinics in Richmond and Martinez, precepting, and curriculum development. My particular interest is in patient-centered, collaborative care. I have been active with the Chronic (diabetes) Care Collaborative, Centering Pregnancy groups, and HIV support groups, and Reach Out and Read.  Outside of work, I like composting, cooking, seasonal biking to work, what little political involvement I can fit in, and life with my partner Francesca. David Hearst, MD
Family Medicine Registrar

After completing medical school at UCLA and doing my Family Practice residency at Fresno, I worked in Community Health Centers for a few years. I became disillusioned with the politics there and moved to CCRMC and its Health Centers in 1987. I have found this a great place to work. We have a dedicated group of doctors, residents, and staff who work well together towards the common goal of providing the best possible care for our patients. Besides doing family practice at the Brentwood Health Center and precepting in Martinez, I am Chair of the Credentials Committee and the Administrative Affairs Committee and serve on the Medical Executive Committee. When I am not working, I like to backpack, hike, travel to beautiful places, garden, and spend time with my wife and children.


Estela Hernandez, MD
Family and Emergency Medicine

I have been a fulltime faculty in the ED since graduating residency in 2001. I came to CCRMC because of its strong procedural teaching and patient population. I also do occasional OB shifts backing up the intern. Between January 2005 to January 2007, I was a member of the Residency Leadership Group as the Associate Residency Director. I continue to work with the Residency Leadership Group, heading the Interview and Ranking Committee for the last four years. I was born in Mexico and immigrated to the US at 11 years of age settling down in Santa Ana, California. I attended Smith College in Massachusetts and then UC Davis Medical School. I enjoy spending time with family, friends, but especially my dog. I also love to cook and dance particularly salsa, cumbia, and rancheras.


Troy Kaji, MD
Faculty, Department of Family Practice

I joined the Family Practice Department at CCRMC in 2000. I graduated from UC Davis School of Medicine in 1986, where I also completed my intern year, followed by 5 years directing a community clinic in Sacramento. I completed my Family Practice residency at Glendale Family Practice Residency Program in 1994, worked at USC Medical School for a year, then did HMO practice in Burbank for 5 years. I then was medical director at St. Anthony Foundation, San Francisco - a free community clinic for the homeless in the Tenderloin area, the year before coming to Contra Costa. I am particularly involved in piloting group visits for Laotian patients, pushing for systemwide implementation of the Chronic Care model to improve care of our diabetics, and helping standardize our approach to treating patients with co-occurring chronic pain and addiction problems. Outside of work I enjoy eating at ethnic restaurants, reading history books, and spending time with my wife and 5 daughters.


Ken Katzman, MD

I graduated from the UCSF - Fresno Family Medicine Residency in 1995 and served an additional year on faculty before moving to Contra Costa in 1996. I completed the UCSF Faculty Development Fellowship in 1997 while working at the Richmond Health Center, then moved my primary care site to Martinez Health Center in 1998. I serve on the Residency Leadership Group as the Educational Director of the Martinez Family Medicine Center. I supervise the Family Medicine staff inpatient services, coordinate the Health Systems Management curriculum, direct the Martinez Health Center Pregnancy Centering Groups, and staff weekly prenatal clinics in Martinez. My passions are Maternal/Child and Adolescent Health. My wife Lisa is a Pediatric Physical Therapist, Early Intervention Specialist and Certified Instructor of Infant Massage. John Lee, MD
Family Medicine Registrar

I came here to CCRMC as a resident in 1972. I coordinate the Problem Based Learning program, and teach prevention, evidence-based medicine, dermatology and computer-assisted learning. I majored in social sciences at Stanford and graduated from medical school at UCSF. I spent two years in the Indian Health Service on the Navajo Reservation (Tuba City). I enjoy films, history, singing/guitar, and basketball.

I started medical school in the mid-1980's after working in public health and primary health care internationally at the UCSF-UCB Joint Medical Program. I completed my family medicine residency at Natividad Medical Center in Salinas and then practiced rural family medicine in California and Hawaii for the next 13 years. My practice then included full-spectrum care including geriatrics, pediatrics, outpatient procedures and high-risk obstetrics including c-sections. I have been on staff here since early 2003 and part of the residency leadership group since 2005. The fellowship is focused on global health and underserved health care, and I often travel abroad to work on health care projects and family medicine/health manpower planning. I am married to Christina and have 3 children, 2 grandchildren, 2 dogs, 2 cats, two cockatiels, and one rat.


Rich McNabb
Family Medicine Registrar

I came to CCRMC as Residency Director in 1990-1998 after serving as Assoc. RD at the Modesto Residency for 8 years. Previous to that, I completed an FP Fellowship at the Univ. of Washington in Seattle and a brief stint in private practice in a small logging town in Oregon. Since being residency director here, I have continued to enjoy the opportunities of a rich clinical practice, teaching the finest residents in the world, and serving as Division Head for the Central County (Martinez) of the Department of Family Medicine. Apart from the rigors of medicine, I try to sustain my faith and strong family/friend ties.

I have watched the CCRMC Residency grow since 1988. While in graduate school at the University of California at San Francisco, I trained in the Family Practice Residency Program in Santa Rosa. My goal at CCRMC is to support residents as they learn how to do a challenging job better, and to facilitate their learning to incorporate tools of working with families, motivational interviewing, and a chronic care perspective. In addition to working at CCRMC, I enjoy hiking and camping with my family, swimming, watching circus arts, and playing Renaissance music.


Tai Roe, MD, Ph.D.

I stayed here at Contra Costa county after graduating from this residency in 2005. Since then, I have increased my opportunities to teach and be involved with the residency as much as possible, because I love teaching. Since October, 2007, I have been a lead preceptor, which means I am involved in development of our outpatient family practice clinic training and do additional precepting. I have also been a member of our residency leadership group since July 2008. Before becoming a doctor, I studied physics in college, got a Ph.D. in molecular and cellular biology, did postdoctoral research and then worked at a genome database project. My beautiful wife and I are expecting our first child at the end of November.


Ken Saffier MD

As a family physician with an interest in addiction medicine, I joined Contra Costa Health Services in 1987. After completing my family medicine residency at Cook County Hospital, and more recently a faculty development fellowship at USC, I've been enthusiastic about teaching, especially experiential learning and enjoy exploring ways to make medical education more effective. Despite the challenges of working in our publicly underfunded health care system, l see our county as being a prototype for a future, desperately needed, national health insurance. When not working, I enjoy fishing, hiking and learning from my grandson.


Alan Siegel, MD
Family Medicine Registrar

I joined the CCRMC faculty after working in private practice and locum tenens for a few years. I graduated from Emory University School of Medicine in 1994 and completed my residency at the University of Virginia in 1997. I recently completed the Faculty Development Fellowship at UCSF. I have always had an interest in residency education and resident wellbeing and this fellowship was a great opportunity to pursue these skills. I am also interested in evidence based medicine & information mastery, which were emphasized in my own residency program, and I am helping to expand the EBM focus at CCRMC. Other interests within medicine include gastroenterology and obstetrics. Outside of work, I play guitar, am involved in a community leadership program, participate in triathlons and century bike rides, and enjoy being outdoors as much as possible. Jon Stanger, MD, MHS, MDiv

I am proud to have been a member of the teaching faculty at CCRMC for the past twenty-seven years. I am a graduate of Stanford University and UCSF School of Medicine. I received a Master of Health Services from UC Davis, Master of Divinity from the Graduate Theological Union in Berkeley, and Certificate in Health Care Ethics from the University of Washington. I am double-boarded in Family Practice and Emergency Medicine, and taught emergency medicine to family medicine residents for over twenty years. My current responsibilities include coordination of the Medical Ethics & Humanities curriculum, as well as chairing our Bioethics Committee. I teach medical ethics at CCRMC, the Graduate Theological Union, UCSF, and David Grant Medical Center. Outside of medicine, my special passions include family, storm chasing, and baseball. Sonia Sutherland, MD
Past Medical Staff President

I joined the staff in 1994 because of my interest in working with the underserved along with my commitment to resident education. My residency training was completed in Internal Medicine at Alameda County Medical Center in Oakland, California. My undergraduate studies were completed at Stanford University and I attended the Drew/UCLA School of Medicine. My primary duties on staff at CCRMC are teaching on the Inpatient Medicine Service, Primary Care at the Richmond Health Center, and am actively involved in Medical Staff and Quality Improvement. I am also one of two Northern California Regional Physicians for Hansen's Disease and work in our Hansen's Disease Clinic. Ori Tzvieli, MD, MS

I joined the faculty in 2003 after graduating from our residency. I work as an inpatient registrar on the Internal Medicine service, and as an outpatient registrar in the Family Medicine department in Concord, and I have a growing medical acupuncture practice. I really enjoy working in a place where people are friendly, kind and compassionate. My other interests include motivational interviewing, meditation, and finding connection and meaning in every moment.


David MacDonald, MD

After finishing medical school at UC Irvine in 1990, I came to the oasis of family medicine residencies at Contra Costa Regional Medical Center. The program provided me with excellent training in the broadest sense for family medicine. There was also a great deal of depth in experience, especially with OB, Internal Medicine and Surgery. I finished the program in 1993 and stayed on as an attending in the department of surgery. Aside from teaching on the surgery service, I have clinics in family medicine, surgery and orthopedics.


David Pepper, MD

A San Franciscan by birth, I attended UCB before moving to the midwest (MD-PhD program) in Iowa which introduced me to Family Practice and primary care. After a FP Residency closer to home (UCSF-Fresno) I joined the UCSF Faculty and stayed in the heart of California for a dozen years teaching and practicing full spectrum family medicine (OB, ER, Inpatient, Clinics, Procedures and Adminstration).

In 2005 I returned to San Francisco to help run a Community Clinic (Mission Neighborhood Health Center), before joining the CCRMC Faculty as a Lead Preceptor in 2007. My academic passions are decision making, computers/graphics/tough cases/PBL and MAPping (ask me.....) and learning. Outside work I enjoy backpacking, hiking, bicycle riding and gardening as well as music, cooking, my three (mostly grown) children, family and friends.

I started with CCRMC as a psychology intern and joined the staff in 2001. I attended the University of Virginia as an undergraduate and completed a doctoral program at California School of Professional Psychology. My current interests include Integrative Medicine and developing group medical visits for Chronic Pain. I am also involved in resident education as a member of the Residency Leadership Group and have a strong interest in resident well-being at CCRMC. Outside of work, I love being a mom, spending time with family and friends, camping and reading novels.


Sumita Kalra, DO

Having recently completed my residency training in Modesto, CA, I recently joined the Department of Family Medicine in September 2007. I completed my undergraduate training at UC Davis and medical school at Touro University College of Osteopathic Medicine. I was drawn to Contra Costa because of the ability to practice medicine as a true family doctor. Currently, I work at RHC where I have my FMC, precept residents, and run a Flexible Sigmoidoscopy clinic. Since joining CCHS, I have found more interest in Chronic Care Disease Management and resident education. This past July I joined the Faculty Leadership Group to further my interest in resident education. When I am not working, I enjoy spending time with my husband, Sumit, and my family.


Krista Farey, MD, MS

I have been happily working for CCRMC and Richmond Health Center since 1989, long enough to enjoy growing up with a generation of continuity patients, and to have had the opportunity to provide obstetrical care for young women I have been caring for since before they were born.  I graduated from the UCB-UCSF Joint Medical Program in 1984, and the UMass Worcester Family Practice Residency in 1987.  My interests include single-payer health care policy reform, group medical visits and integrative medicine. I've served as president of the medical staff, and of California Physician's Alliance.  I train residents in group prenatal and group pediatric care.  I live and play with my husband Vishu, our two teenaged daughters, Usha and Anuradha, a black cat, a backyard labyrinth, and world music flutes and drums.
