Calling To Schedule An Appointment?

Please note the following important changes to our phone system

Effective July 1, 2003

"You have reached the Contra Costa Health Centers' Patient Access Line....for appointments, press 1"

When you call our Patient Access Line at 1-800-495-8885 to schedule an appointment, you will find that our phone system has changed. You will be asked to use your touch-tone phone to enter a number that identifies the patient who needs an appointment. When you accurately enter one of the numbers requested, your eligibility for an appointment will be determined and your call will be routed to the person who can best help you. If you do not have a touch tone phone, an operator will pick up your call. The following describes the new options on the phone tree:

Actual card is cranberry colored. Press 1 if the appointment is for a person who has a medical record with Contra Costa Health Services..

Enter the patient's medical record number (in the box at right) found on their clinic card followed by the # sign or press 9 for further instructions. Be sure to enter all nine numbers, including the number after the hyphen.

Actual card is light blue with black lettering. Press 2 if the appointment is for a Contra Costa Health Plan (CCHP) Member.

Enter the member's CCHP Member ID number (in the box at right) followed by the # sign or press 9 for further instructions.

NOTE: CCHP members should receive new cards in early June. If you have not received your new card by July 1st, 2003, please call CCHP Member Services at 1-877-661-6230 (option 2) to order cards for you and your family.

Actual card is white with blue letters on front. Press 3 if the appointment is for someone who has applied for or has Medi-Cal coverage.

Enter the patient's Medi-Cal ID number (in the box at right) found on their Medi-Cal card followed by the # sign or press 9 for further instructions.

Press 4 to speak with a financial counselor.
