This is the process that the family has to follow:

  1. Client contacts dental office and arranges an appointment for evaluation.
  2. If the dentist recommends sedation, the client needs to call the CCHS Children's Oral Health Program office and speak to Tracy Hopkins at 925-313-6157. Tracy is the care coordinator and she will need to ask the family some financial eligibility questions.
  3. If family meets financial eligibility, Tracy calls the dental office and requests a treatment plan.
  4. After receipt of a treatment plan, Tracy will request assistance from the President of the Dental Foundation** requesting help with payment of sedation fee.
  5. Once approved, Tracy faxes approval to the dental provider.
  6. Tracy calls the family to let them know that it has been approved.
  7. The family has to call the dentist to set up the next appointment.

**Medi-cal does not pay the sedation fee but the dental foundation will pay $150 of the sedation fee. Dr. Salazar charges $200.

Dr. Williams charges $160 and waives the $10 family balance if they cannot pay it. Dr. Aubushom charges $100.
