Services for People with HIV and Their Families


The Contra Costa System of Care provides services for people with HIV to increase access to medical care and to stay as healthy and independent as possible. The system is centered around case management services that work with clients based on their level of health.

The system consists of Home-based and Clinic-based Short Term Assistance, Community-based Case Management, and Nursing Case Management. Referrals are provided for HIV specific support services and include housing assistance, legal services, benefits advocacy, substance abuse, mental health, food, transportation, home care, emergency assistance, day center service, other programs (such as AIDS Drug Assistance Program, Women's Early Intervention Program, and Prevention with Positive Program), and links with the System of HIV Prevention.

Our Intake Coordinator serves as a centralized source of information and referral for HIV+ people in Contra Costa County. If you or someone you know is HIV+ and interested in finding out more about services in our county, call John Sturr, MSW, at 925-313-6781.

Short Term Assistance

Medical Social Workers assist people with HIV by addressing their most immediate needs and connecting them with ongoing case management services. Our team's Public Health Nurses and Medical Social Workers provide Clinic-based services at the Center for Health in North Richmond, Richmond Health Center, and Contra Costa Regional Medical Center, including linkages to case management, counseling, education and prevention services, assistance with partner counseling and access to medical care. Home-based services are also available.

Case Management

Community-based Case Managers provide ongoing psychosocial support for people with HIV, whose health statuses do not require the services of a nurse case manager.

Nurse Case Management

A Public Health Nurse and a Medical Social Worker work as a team to provide case management services to people with severely disabling HIV or AIDS. Our goal is to assist clients in remaining independent and at home as long as safely possible. The case managers coordinate services including medical care, attendant care, nutritional counseling, mental health, substance abuse, and home delivered meals.

AIDS Drug Assistance Program (ADAP)

This program pays for medications that treat HIV disease. In order to be eligible for ADAP, a person must be HIV positive, a California resident over the age of 18, have limited or no prescription benefits, and meet financial eligibility requirements. Women's Early Intervention Program (WEIP)

Many women who are living with HIV and AIDS feel alone and isolated. The Women's Early Intervention Program provides psychosocial support, health education, and risk reduction services to help keep HIV + women in East and Central County healthy. Women with greater immediate medical needs are offered Nurse Case Management services. Pamela Phillips, MSW, is the Program Coordinator for WEIP and can be reached at 925-313-6958.

Prevention with Positives (PWP)

Prevention with Positives (PWP) helps people with HIV make a plan to prevent transmission of HIV to others while maintaining their own health. Referrals are made based on individuals' needs. Derelle Hill at 925-313-6604 and Maxine Little at 925-313-6605 are Risk Reduction Specialists.
