Permanent Connections

Permanent Connections provides subsidized permanent housing linked to services for homeless youth with chronic mental illness, HIV/AIDS, developmental, or physical disabilities. Youth over the age of 18 receive "wrap-around" support services that assist them to not only maintain, but also thrive in their housing.
