Project Coming HomePurposeThe Homeless Outreach Project to Encampments (HOPE), Health, Housing and Integrated Services Network (HHISN), and Shelter Plus Care programs work together as Project Coming Home (PCH), a permanent, supportive housing program for chronically homeless, disabled individuals, including veterans. The goal is to end an individuals homelessness using a "housing first" approach to immediately place individuals in permanent housing and provide wrap around support services. What we offerOutreach teams go out to encampments throughout the county to assist the homeless access services; multi-disciplinary supportive service teams supportive services teams tied to permanent housing using Shelter Plus Care rental assistance. EligibilityChronic homeless individuals with a mental health condition, physical illness or substance abuse disability. Must have been homeless for a year or longer or have at least four episodes of homelessness in the last three years.
