Project Coming Home - Addressing Addictions to Alcohol (AAA)PurposeThis program is an expansion of Project Coming Home that provides permanent housing and supportive services to chronically homeless individuals who have a long-term addiction to alcohol and will strengthen and formalize PCH's existing collaboration with County law enforcement and the court system What we offerOutreach teams that go out to encampments throughout the county to assist the homeless access services; multi-disciplinary supportive services teams tied to permanent housing using rental assistance. EligibilityHomeless individuals with a mental health condition, physical illness or substance abuse disability. Must have been homeless for a year or longer or have at least four episodes of homelessness in the last three years.
