Families Forward was initiated in 2007 and will serve up to 100 children and their families in Far East County. The target population is unserved children 0-18 years of age in Far East County who have a history of repeated failure in learning environments, including childcare, preschool, home, and school and who are from families at or below 300% of poverty level and not eligible for other funding sources. Priority within this population will go to those with one or more of the following risk factors: Multiples foster care or family caregiver placements, limited English proficiency, high risk parents or community, populations whose cultural differences have traditionally excluded them from mental health services, trauma, substance abuse, experience with the juvenile justice system, or experience with Psychiatric Emergency Services. This is a Full Service Partnership. See the CSS Plan for full description of services and supports. The new Transition Age Youth Program (ages 16-25) focuses on the West county area, and serves young adults with serious emotional disturbances or serious mental illness, who are homeless or at imminent risk of homelessness. 135 transition-aged youth will be served in the next two years. Housing supports are an important element of our Full Service Partnerships and of our recovery philosophy. Housing for Full Service Partners are being developed through a collaborative effort between CCMH and Shelter Inc. Additionally, CCMH is working with the Contra Costa County Community Development Department to award one-time MHSA funds for efforts that increase the number of supported housing units available in the county for mental health consumers. $1.6 million was available for this effort during an RFP process in 2006. To-date, funds have been awarded to Rubicon Programs, Villa Vasconcellas Senior Housing and the Lillie Mae Jones Plaza. Additional funds will become available in the future from a State-led MHSA housing initiative.

The Bridges To Home Full Service Partnership is now serving West County adults with serious mental illness, who are homeless or at serious risk of homelessness, and who are not currently being served by County Outpatient Mental Health Services. 150 adults will be served in the next two years. Bridges To Home is a collaboration with Rubicon, Familias Unidas, Asian Pacific Psychological Services, Anka Behavioral Healthcare, Mental Health Consumer Concerns and County Mental Health. The Older Adult program will begin in early 2008, and will serve seniors countywide with teams providing outreach, engagement and on-going services to the most imperiled seniors in the county. Services will be both community and home-based. CCMH will also pilot the Impact Program model in primary care clinics in each county region. Short-term cognitive therapy will be provided in the clinics to older adults with medical diagnoses of diabetes, cardio-vascular disease and/or chronic pain and who show signs of depression and possible suicide. Both outreach and clinic-based services will lay groundwork for more comprehensive services and increased service volume in the future. 425 clients will be assessed in the first year with 225 of those clients becoming active for ongoing care management. These efforts are Systems Development Strategies.
