Community Health Assessment (2025)
The Public Health Community Health Assessment (CHA) is currently underway, with planning efforts being led by a Steering Committee.
A CHA is a structured, data-driven process used by public health departments, hospitals, and community organizations to assess the health status, needs, and challenges of a specific population. It serves as a critical foundation for improving public health outcomes and advancing health equity.
This collaborative effort involves strong partnerships with community members and organizations, and relies on comprehensive, broad-based data to identify and describe key health issues affecting the community.
