Across Contra Costa, community partners are coming together to better understand how residents are doing, what helps boost well-being, and what may be making well-being harder to reach. At the heart of this Community Health Assessment is a commitment to ensure voices from across our communities are heard, valued, and reflected in what is shaped, prioritized, and invested in next.

Background Information

About the CHA

The Community Health Assessment (CHA) is an important effort led by the Public Health Department within Contra Costa Health (CCH) to better understand the health needs of our communities throughout Contra Costa County, and advance health equity. Guided by a Steering Committee made up of county staff, community‑based organizations, and local health institutions/Medi-Cal providers, the CHA uses both primary data (surveys, listening sessions, focus groups, etc.) and secondary data (existing health and demographic information) to build a full picture of community health.

To hear from as many residents as possible, a survey in 16 languages is being launched and CCH is supporting listening sessions across Contra Costa County, with a focus on reaching communities that are most vulnerable or historically underrepresented, as part of primary data collection.

The findings from the analysis of the primary and secondary data will be shared in a CHA report on the CCH website and will help shape the Community Health Improvement Plan (CHIP). This work reflects our commitment to listening to community voices and using what we learn to guide the future of health in Contra Costa County.

Why this survey matters

We believe in a future where all people and places are thriving, no exceptions. Reaching that future means looking beyond the numbers to understand what people are facing, what strengths already exist, and where change is needed most.

Data can help show patterns, but community voice helps reveal what is behind them. When we pair lived experience with local, state, and national data, we gain a fuller picture of well-being and the vital conditions shaping daily life across Contra Costa.

Who is stewarding the work

This Community Health Assessment is led by Contra Costa Health (CCH) and shaped in partnership with a Steering Committee made up of county staff, community‑based organizations, and local health institutions/Medi-Cal providers.

CCH is also working with Contra Costa Together, which is helping broaden outreach by connecting partners, sharing the survey, and increasing participation across the county.

Together with the Steering Committee, Contra Costa Together, and other community partners, CCH aims to reach as many communities as possible and engage residents from all corners of Contra Costa County—especially those whose voices have historically been underrepresented.
