Program Description
Older Adult Mental Health provides a continuum of care to seniors, who are age 60 and above, throughout Contra Costa County with teams that provide outreach, engagement, and on-going services to the most imperiled seniors in the county.
Our Intensive Care Management Program consists of three multidisciplinary teams who provide field-based services to individuals diagnosed with a chronic and severe mental illness in their homes, board and care homes, shelters, etc. The Intern will be working with a multi-disciplinary team, which includes a Geropsychiatrist, Nurse, Mental Health Clinicians and a Community Support Worker. The intern will gain experience in several areas, including providing case management services to older adults, collaborating with family members and other service providers, and providing crisis intervention/risk assessment as needed. In addition, there can be the opportunity to complete intakes, and provide supportive therapy.
Older Adult Mental Health utilizes the IMPACT Program model in Behavioral Health clinics in each region of the county. Short-term cognitive behavioral therapy is provided in the clinics to older adults, who often have co-occurring medical diagnoses such as diabetes, cardio-vascular disease, and/ or chronic pain as well as moderate to severe mental illness, including depression, anxiety or Post-Traumatic Stress Disorder. This program consists of a multi-disciplinary team, which includes a Geropsychiatrist, Mental Health Clinicians, and Community Support Workers. The intern will gain experience in several areas, including providing intakes and therapy to older adults using Evidence-Based Practices. Referrals to community agencies, Alcohol and Other Drugs services, education, and collaboration with family members and natural supports will also be part of the job duties.
This is a field-based position, which will be primarily located in Central County, and may require travel to East and/or West County regions. A stipend will be provided by Contra Costa County.
