How to Sign Up for MyChart
You have a few different options to activate your own MyChart account.
Self-signup: Go to the signup page on the MyChart website and fill out the requested information. Once we match the information with our records, you will be on your way!
At your doctor's office: If you don't have MyChart yet, you will automatically get an activation link by text message or email when you check in for your visit.
Contact MyChart customer support: Call us toll-free (1-844-622-5465) or send us an email at MyChart.support@cchealth.org saying you'd like to start a new MyChart account. After we confirm your identity, we will send you an email with an activation code. That's it!
