Skilled Nursing Facilities

• COVID-19 outbreaks: Shared Portal for Outbreak Tracking (SPOT)
• All other outbreaks: Report an Outbreak (non-COVID) to Public Health

 On both forms you will be asked to enter information about the outbreak at your location.  Then you will be asked to enter information about the cases you are reporting. Once the information you enter is reviewed by the health department, you will receive a call and/or email from a Public Health Nurse to follow-up and provide guidance. If needed, you will also receive a shared excel spreadsheet so you can more easily share updates with the health department about new developments with your outbreak.  

Providers and Administrators can report electronically via CalREDIE (California Reportable Disease Information Exchange) Provider Portal (PP) to comply with provider disease reporting requirements as outlined in Title 17 CCR §2500.

Alternatively, this form, Communicable Diseases other than TB and HIV/AIDS (110a) can be completed and faxed to us at 925-313-6465.

Contra Costa Health evaluates Skilled Nursing Facilities on 10 Criteria. Facilities that meet 9 or more of the criteria earn the Skilled Nursing Facility Public Health Preparedness and Safety Certificate. The certificate is issued twice a year. 

Criteria 

1. Opens emails from Acute Communicable Disease department

2. Attends quarterly Infection Prevention and Control meetings 

3. Regularly engages assigned Public Health Nurse 

4. Updates Acute Communicable Disease department with staff contact information

5. Receives California Health Alert Network messages 

6. Uses appropriate methods to report Outbreaks (SPOT, MS Forms, Sharepoint) 

7. Participates in/hosts training sessions

8. Has a plan for respiratory pathogen vaccination of staff and residents

9. Facility infection prevention nurse has a designated trained backup

10. Promotes use of Healthcare Facilities Transfer Form
