A Culture of Caring for More than 50 Years

Contra Costa Health Plan (CCHP) is a county-sponsored health-maintenance organization (or HMO) that provides managed care for more than 200,000 people in the county.  


CCHP is the primary managed-care provider for Medi-Cal beneficiaries in Contra Costa and we also manage smaller plans for county employees and IHSS homecare workers.  

Our members have access to hundreds of family medicine doctors and specialists in our provider networks. CCHP members also enjoy a wide array of medical benefits as part of their plans from primary care to specialty care to dental services to prescription medications.

Accreditation  

CCHP is accredited by the National Committee for Quality Assurance (NCQA), an independent body that conducts rigorous assessments of health plans’ structures and processes, clinical quality and patient satisfaction. CCHP is only one of six Medicaid managed-care plans in California to receive a four-star rating from NCQA.  

Delegated Credentialing Model

CCHP has a delegation relationship for credentialing of some of our contracted providers with several health care systems, which include Stanford Healthcare, University of San Francisco Medical Group, John Muir Physician Network, Lucile Salter Packard Children’s Hospital and Medical Group, BayChildren’s Physicians, and Contra Costa Reginal Medical Center. Our delegated entities must comply with California law, CCHP policies, and National Committee for Quality Assurance (NCQA) standards. CCHP monitors and oversees these delegated credentialing and recredentialing activities to ensure members receive qualified medical care. CCHP holds ultimate responsibility for ensuring the quality of medical care provided by delegated entities.

History 

CCHP has been serving the health needs for people in the County for more than 50 years. CCHP was the first federally-qualified, state-licensed, county-sponsored HMO in the United States. In 1973, we became the first county-sponsored health plan in California to offer Medi-Cal managed care coverage and in 1976, we became the first county-run HMO to serve Medicare beneficiaries. In the 1980's, we expanded our programs to reach out to county employees, businesses, individuals and families.

Annual Network Certification

The California Department of Health Care Services (DHCS) requires Medi-Cal Managed Care Health Plans like the CCHP to conduct Annual Network Certifications to validate network adequacy. Here is our most recent Network Adequacy Report.

CCHP Alternative Access Standards

DHCS sets time and distance standards to ensure health plan members can access certain health care services within specific mileage or travel time from their residence, or via telehealth for approved services. Health plans must comply with these standards or obtain DHCS approval for an Alternative Access Standard (AAS). Visit our AAS webpage.
