Contra Costa Health Plan (CCHP) brings clinical and community voices together to guide quality and equity across our Medi-Cal programs. This work is led by the Quality Improvement and Health Equity Committee (QIHEC), a partnership between two standing councils: the Quality Council and the Equity Council.

Purpose

The councils support the planning, oversight, and ongoing development of CCHP’s Quality Improvement and Health Equity Program. They help ensure that both quality and equity goals are integrated across initiatives, from clinical performance to member experience.

Both councils are co-chaired by CCHP’s Chief Medical Officer and the Director of Quality and Health Equity, who work together to connect the councils’ work to overall health plan operations and executive leadership. The Quality Council brings a clinical lens, while the Equity Council adds community and public health perspectives. Together, they ensure decisions are coordinated and responsive to both quality and equity priorities.

Responsibilities

Review and provide feedback on the Quality and Health Equity Annual Work Plan, Program Description, and Year-End Evaluation
Oversee quality and equity-related initiatives, including HEDIS measures, performance improvement projects, and population health activities
Monitor member experience through surveys, grievances, and appeals with attention to both quality and health equity concerns
Review clinical guidelines, utilization trends, and population health data to identify opportunities for improvement
Recommend and track corrective actions when performance gaps or disparities are identified
Ensure that provider and community perspectives inform planning and accountability at all levels
