Contra Costa Health Care Plus (HMO D-SNP)

Care Plus makes health care simple by bringing your Medicare and Medi-Cal benefits together in one easy plan. You’ll have one plan, one membership card, and one phone number to call. 

With Care Plus you have a personal care manager who helps you get the care, medicine and extra benefits you need to stay healthy without the hassle. More support, less stress—Care Plus delivers care that truly adds up for you.

What is Covered?

You’ll get the following care with zero premiums, deductibles, and no to low co-pays:

• Individual care plans and the support of a dedicated care manager
• Medical and hospital care with a large network of providers
• Certain prescription medications, including Part D prescription coverage
• Preventive care, such as wellness exams and vaccines
• Low or no co-pays on prescription drugs
• Mental and behavioral health care
• Managed long-term services and supports
• Vision care, including an annual exam and $200 towards glasses or contacts every year
• Everyday over-the-counter (OTC) health items, like first aid supplies and medicines
• Prepaid grocery card for members with eligible chronic conditions
• All Medicaid services, including transportation to and from appointments, and all community support services

Who Is Eligible?

Care Plus is a Dual Eligible Special Needs Plan for low-income seniors and people with disabilities who are enrolled in both Medicare and Medi-Cal.

To join Care Plus, you must:

• Live in Contra Costa County
• Have Medicare Parts A and B
• Have full-scope Medi-Cal
• Be age 21 or older
