Contra Costa Health Care Plus (HMO D-SNP) is a Dual Special Needs Plan that combines Medicare, Medi-Cal and prescription drug benefits into one plan. It is designed only for people living in Contra Costa County who are enrolled in both Medi-Cal and Medicare.

With Care Plus, you are assigned a personal care manager to help you get full access to the care, benefits and prescriptions you need with less work for you.

Care Plus is a Dual Special Needs Plan for low-income seniors and people with disabilities who are enrolled in both Medicare and Medi-Cal.

To join Care Plus, you must:

• Live in Contra Costa County
• Have Medicare Parts A and B
• Have full-scope Medi-Cal
• Be age 21 or older

There is no cost to join Care Plus. You will not pay premiums, deductibles or co-pays or co-insurance for medical services. Some prescription medicines may have co-payments.

Care Plus offers the benefits of Medicare, Medi-Cal and more all in one plan. Care Plus includes an individual care plan and the support of a care manager, medical and hospital care, prescription medicine, everyday over-the-counter health items, preventive care, mental and behavioral health care, managed long-term care services and supports, a prepaid grocery card for members with eligible chronic conditions, and more.

Yes, Care Plus provides Part D (prescription drug) benefits at no cost to you. Prescription medicines are covered based on an approved formulary which may change from year to year. Some medicines may require prior authorization, step therapy or have quantity limits. Everyday over-the-counter (OTC) health items, like first aid supplies and medicines, are also covered.
