Your personal care manager makes sure you get the care, benefits and prescriptions you need, with less work for you. Care Plus brings everything together, delivering care that truly adds up for you.

What Is Covered?

No copays, premiums or deductibles for care

This includes doctor and hospital visits, lab tests, vaccines, specialists, behavioral health therapists and more.

       Care management

You will be matched with a care manager who works with you to create a care plan and coordinates all aspects of your care. They also help you find doctors and specialists, make health care appointments, set up no-cost rides to health care visits, get follow-up care and connect you with programs such as home health care, housing supports, meal programs and more.

 

Prescription medicine coverage

Including $0 premium, low or no copays on generic and brand-name medicines. Check if the medicines you use are covered.

 

Vision Care

Including a free annual exam and $200 towards eyeglasses or contact lenses every year.

 

Flex Card

For over-the-counter health items, like first aid supplies and medicines and a prepaid grocery card for members with eligible chronic conditions. 

 

Extra benefits

Including free rides to approved healthcare appointments and the pharmacy, everyday over-the-counter health items, connections to social groups, and a prepaid grocery card (for members with eligible chronic conditions).
