Search for Covered Drugs (Formulary)

 * Medi-Cal members: You or your provider should contact Medi-Cal Rx.   The Medi-Cal pharmacy benefit is not managed by CCHP.

Pharmacy Overview

  1. All CCHP Commercial Plan members and recipients of county health programs are subject to the Preferred Drug List (PDL) protocols.  
  2. All prescriptions must be filled at a CCHP network pharmacy. 
  3. Contact the CCHP pharmacy unit at 1-877-661-6230 (Option 3) (TTY 711) if you: 
    • Have questions regarding the formulary, covered drugs, prior authorizations, or other pharmacy related matters.
    • Need information about the Formulary or Preferred Drug List (PDL) in your language.
  4. For general information about pharmacy covearge and co-payments, refer to your Member Handbook / Evidence of Coverage.  For real time pharmacy benefits information (e.g., coverage of specific drugs and co-payments) create an account and log into the PerformRx portal https://cchp.darwinrx.com/members

The Formulary or Preferred Drug List (PDL) is only a listing of preferred medications.  If your provider has clinical reasons to choose a second line agent, they will need to complete a prior authorization form and contact the health plan. 

 Most over-the-counter (OTC) medication is not covered, except where specified, and medical supplies and devices are not included in the PDL.

Closure of Rite Aid Pharmacies

In May 2025, Rite Aid announced that it would be closing numerous pharmacies across multiple states as it moves through bankruptcy proceedings.  Some of these pharmacy closures may affect CCHP members.

Please remember that CCHP has many pharmacies in our pharmacy network. Almost all of the major pharmacy chains (including CVS, Walgreens, Costco, Safeway and Walmart) are in the pharmacy network as well as many independent retail pharmacies.

Mail Order Prescription Service

Our Pharmacy Benefits Manager, Perform Rx, has a contract with Walgreens to allow members in the plans listed below, to sign up to receive certain medications by mail with a valid prescription.

Plan Name:

  • County Plan A/B, Plan A2
  • IHSS Plan A2
  • ARCC Plan
  • Superior Court Plan

By registering with the Walgreens Mail Service website you can conveniently refill your prescriptions online, access your prescription history order status, and more.
